Predicting molecular biomarkers / outcomes from H&E whole-slide images

Predict molecular status (MSI/MMR, HRD, BRCA and other actionable mutations, gene-expression programs) or clinical outcome (recurrence, treatment response) directly from routine H&E whole-slide images, which every cancer patient already has, instead of waiting for and paying for sequencing or IHC. Commercially it promises cheap, fast triage for targeted therapy and companion diagnostics and is the business of Owkin's diagnostics spin-out Waiv, Paige, PathAI and the pathology foundation-model makers (Bioptimus, Owkin Phikon). Today's deployed products are pre-screening tools that rule out negatives and send the rest to molecular testing; a 'solved' bar is accuracy good enough to replace the molecular test for a given biomarker (e.g. sensitivity and specificity >= 95% on external multi-site cohorts, the level needed for a stand-alone companion diagnostic). A continuous proxy is the HEST-Benchmark, where the best foundation model reaches only ~0.42 mean Pearson correlation for spatial gene expression predicted from H&E.

Why it is still open

On the public HEST-Benchmark (9 cancer/organ tasks, 50 genes each, predicted from H&E patches) the best of 25 pathology foundation models, Bioptimus H-Optimus-1, averages Pearson 0.4229 (ResNet50 baseline 0.3252) as of Apr 2026; deployed H&E-to-MSI products such as MSIntuit are validated as pre-screening tools, not replacements for molecular testing.

Who is working on it

Waiv (formerly Owkin Dx)undisclosed Owkin spin-out selling AI tests that read biomarkers from H&E: MSIntuit suite (MSI), RlapsRisk BC (breast cancer relapse risk), BRCAura (RUO BRCA); Daiichi Sankyo ADC biomarker collaboration May 2026.
Paigeundisclosed Pathology AI company with Virchow/PRISM2 foundation models and OmniScreen, which screens for gene alterations directly from H&E whole-slide images.
Owkin$308M Built MSIntuit (MSI pre-screening from colorectal H&E) and a gBRCA digital-pathology screen with AstraZeneca, released Phikon/Phikon-v2 pathology models; now centres on the K Pro AI agent and spun diagnostics out as Waiv. InvestorsSanofi
Bioptimus$76M Builds pathology/biology foundation models (H-optimus-0/1, M-Optimus) whose embeddings top the HEST-Benchmark for predicting gene expression from H&E; biomarker prediction is a main downstream use. InvestorsCathay Innovation, Sofinnova Partners, Bpifrance (Large Venture Fund), Andera Partners, Hitachi Ventures, Boom Capital Ventures, Thomas Wolf (angel)
PathAIundisclosed AISight platform with AI biomarker quantification (AIM-HER2, AIM-PD-L1, AIM-ER/PR/Ki67, AIM-MASH with FDA/EMA qualification for MASH trials); mostly quantifies IHC/histology rather than inferring molecular status from H&E.

# Predicting molecular biomarkers from H&E whole-slide images

Example targets: MSI/dMMR status in colorectal cancer, HRD status in breast/ovarian
cancer, ISUP grade (PANDA). This file covers binary biomarkers.

Concrete in Lean: the submission (slide → score), AUROC over a finite labelled set as
the Mann–Whitney statistic (ties count ½), sensitivity at a fixed decision threshold,
and a Prop requiring the bar on EVERY external cohort (not the development cohort).

Opaque oracle (`Cohort`): the hidden slides, their ground-truth labels from the
reference molecular assay (PCR/IHC for MSI, genomic scar assay for HRD), and the
guarantee that cohorts come from institutions/scanners not used in training.
Attesters: the data custodians (hospital consortia / challenge organisers) who hold the
labels and run the frozen model.

Units: AUROC, sensitivity, specificity dimensionless in [0, 1].
Indicative bars (MSI screening literature): AUROC ≥ 0.90, sensitivity ≥ 0.95 at a
pre-registered threshold, specificity ≥ 0.5 there (rule-out use).
